Decree of Adoption - The decree of adoption is issued
by the court of jurisdiction. This document is the final
document in the adoption paperwork. The decree of adoption
is sent to Vital Statistics to prove that an amended birth
certificate should be issued, and also this document is
used to prove that the adopted parents are the new
adopted parents of record. The decree of adoption
many times is readily available. The adopted parents
may have this document, or it may be obtainable from
the county court house of jurisdiction by the adopted
parents. Many times (but not always), the decree of
adoption has the adoptee's original first and last
name.
